Dental Insurance Information

At Lund Dental Associates we make every effort to provide you with top-quality dental care while  helping you maximize your insurance benefits. Because insurance policies vary, we can only estimate your coverage in good faith but cannot guarantee coverage due to the complexities of insurance contracts. Your estimated patient portion must be paid at the time of service. As a service to our patients, we bill insurance companies for services. If your insurance company does not pay the claim within 60 days we will forward the balance to you the patient. If you have any questions, our friendly staff is always available to answer them. The patient is ultimately responsible for the entire balance.

Please contact your insurance company to see if we are listed in their network, and if not ask what your plan’s out of network coverage is.

To help us with your insurance filings, please complete the insurance questions on our questionnaire.  If, during the course of treatment your benefits change, the financially responsible party is accountable for all charges.

Please bring your insurance information with you to the consultation so that we can expedite reimbursement.

Please understand that your dental insurance is a relationship between you the patient and the insurance company.  Your plan you have was decided upon between your employer and the insurance company.
